Blake William Ebo Evans, 24, died at 10:30 PM, Sunday, May 25, 2008, while serving in the Army in Iraq. Blake was born in Rockford on September 21, 1983, the son of Craig and Judy Belk. He married Shannon Sherwood in Kentucky on August 6, 2005. Blake was a Sergeant in the Army serving in Iraq with the 101st Airborne, Delta Company, 2nd BTN, 327th INF, 1st BCT, No Slack. Blake is survived by his wife, Shannon; daughters, Adriana and Kylee; parents, Craig and Judy Belk; brother, Scott Evans all of Rockford; mothers-in-law, Peggy Huaracha of Kentucky and Susan Sweitzer of Rockford; nana, Marion DiGiovanni of Caledonia; paternal grandparents, Bob and Mary Belk of Kentucky; several aunts, uncles, and cousins; his extended military family and his three canine companions, Bella, Gracie and Zoie. Blake is predeceased by his nanu, Thomas DiGiovanni, a WWII Veteran and his great grandparents, special aunt, Carmella and many other aunts and uncles. Mass of Christian Burial will be 10:00 AM, Wednesday, June 4, 2008, in St. Anthony of Padua Catholic Church, 1010 Ferguson St. Rockford, IL 61102 with Fr. William R. Wentink officiating. Entombment in Sunset Memorial Gardens. Visitation will be from 3pm until 8pm Tuesday, June 3, 2008, at Fitzgerald Funeral Home and Crematory, Mulford Chapel, 1860 S. Mulford Rd. Rockford, IL 61108. In lieu of flowers a memorial fund has been established for his children c/o Blake William Evans Childrens Fund at AmCore Bank, 401 7th Street, Rockford, IL 61104.
